What Is Peripheral Neuropathy?
Peripheral neuropathy is damage or dysfunction of the peripheral nerves — the vast network of nerves outside your brain and spinal cord that carries motor commands to your muscles and sensory information back to your brain. When those nerves are damaged, the signals they send become unreliable. Some signals are weakened (causing numbness or weakness). Others fire incorrectly (causing burning, tingling, or pain). Many patients experience both at the same time, often in the feet and lower legs first, and later in the hands.
It’s a startlingly common problem. An estimated 20 million Americans live with some form of peripheral neuropathy — and for many, it goes undiagnosed for years because the symptoms are dismissed as “just getting older.”
Causes of Peripheral Neuropathy
Neuropathy isn’t a single disease — it’s the end result of many possible underlying conditions. Common causes include:
- Diabetes
- Vitamin deficiencies
- Chemotherapy
- Alcoholism
- Prescription medications
- Agent Orange and other environmental toxins
- Degenerative or herniated discs
- Spinal stenosis
- Ischemia
- Rheumatoid arthritis and other autoimmune disorders
- Idiopathic
Identifying the underlying cause is critical because it shapes the treatment plan — diabetic neuropathy and chemotherapy-induced neuropathy may both produce similar foot symptoms but call for different supporting strategies.
Symptoms of Neuropathy
Patients may experience:
- Sharp, stabbing pains or cramps in the feet, legs, or hands
- Burning pain — often worse at night, sometimes severe enough to disrupt sleep
- Extreme sensitivity to touch (even bedsheets can be painful)
- Loss of balance or coordination — frequent stumbles, hesitation on stairs
- Persistent tingling or “pins and needles” sensations
- Weakness in the arms and/or legs
- Complete numbness in the feet — patients may describe a feeling of walking on cardboard or sand
- Discoloration of the feet (paleness, redness, or bluish tint)
- Unusually cold feet or hands even in warm weather
- Unexplained falls
Untreated neuropathy progresses. Over time, the loss of sensation in the feet creates serious risk of unnoticed injuries, infections, and falls — particularly in older adults and people with diabetes.
